Non-Emergency Medical Transportation

The non-emergency medical transpiration program reimburses medical assistance recipients for driving themselves to medical appointments and arranges transportation to medical appointments for individuals who don't have their own vehicle.

Reimbursement

To be reimbursed for your non-emergency medical transportation mileage, please download, print and fill out the form below. Nicollet County has 30 business days to process vouchers. Vouchers received by our agency 90 calendar days after an appointment may not be paid.

Those without a vehicle are transported by a contracted transportation provider. These arrangements are made through our medical transportation coordinator. Due to limited transportation providers, contact with our transportation coordinator needs to be made at least three (3) days before the scheduled appointment to allow time to coordinate with our providers. 

Please note that traveling outside the 30/60 mile transportation limits must be  approved prior by your worker.
