About Home Care Services
Most individuals want to stay in their own home as long as possible. Our program is designed to support you or your family member in an effort to live at home independently. Nurses, in partnership with physicians and other health care professionals work with you to meet your needs.

Home health aides and homemakers are certified, experienced, caring, and trustworthy. We often say they are the heart and soul of the service we provide. Asking for help can be challenging. Clients receiving services often report, "I don't know what I would do without it."

Our goal is to promote quality of life, independence, health and safety in the comfort of your own home.

  • Hair Care
  • Minor treatments under nurse's supervision
  • Skin Care
Homemaker Services
  • Grocery shopping
  • Laundry
  • Light housekeeping and cleaning
Nursing Care
  • Medication instruction and set-up
  • Monitor blood sugar and blood pressure
  • Monitor vital system functions such as heart, lung, and kidney
  • Information about self-care such as medication, exercise, diet, medical conditions
  • Supervision of home health aide or homemaker
  • Support for caregivers
  • Support family members
  • Information and assistance in referral to other community agencies and services
Paying for Home Care Services
Fees for Home Health Services may be paid through Medical Assistance, Waiver Services, and Public Health Nursing Clinic, or privately according to a sliding fee scale based on income. County tax dollars pay for non-reimbursed services.

Home Safety Check-up
Nurse completes home safety walk through evaluations at no charge. Nurses promote the Move More, Fall Less exercise program based on the Minnesota Falls Prevention Initiative. To learn more about the program, read the Home Safety Check-Up (PDF).

Any eligible person in Nicollet County shall have equal access to home care services without regard to race, creed, economic status, color, sex, national origin, physical or mental handicap

The Class A Professional Home Care License granted by the Minnesota Department of Health.
